Lakeside Pediatrics is devoted to keeping our families informed about important changes within the new medical health care laws.
As of July 1, 2024, a new Idaho state law (Idaho Code § 32-1015) requires parental consent before medical services can be provided to minors.
Parents must provide consent for ANY non-emergency health services for their minor children (under 18 years of age).
This includes routine check-ups and treatments.

These documents are available to both new and existing patients.
Lakeside Pediatrics uses an electronic registration system for collecting our new patient paperwork. An online registration link will be sent 5 days before your appointment. If you are unable to complete the online registration beforehand, please be sure to arrive 20 minutes before the appointment time to complete the paperwork in office.
